Waiver - I understand participation in this activity is inherently dangerous and that injury and illness (including communicable diseases such as Influenza, MRSA, Coronavirus, etc.) are possible. I agree to assume the full risk of any injuries, damages, or loss that I or my child(ren) may sustain as a result of participating in any and all activities connected or associated with this program. I agree to waive and relinquish all claims I may have, as a result of my participation in the program, against Great Parks of Hamilton County and their agents, employees, and volunteers. I understand that the Great Parks of Hamilton County, Volunteers, and PGA Instructors assume no responsibility for accident or injury to participants during these activities.
I give my full permission for Great Parks of Hamilton County and their sponsors to use any photographs, videotapes, or other recordings of me or my child(ren) that are made during this program.
Cancellations & Refunds - We will provide a full refund for cancellations made at least 48 hours prior to your scheduled activity. No refund will be given for cancellations made less than 48 hours prior to the activity. If you must cancel with less than 48 hours notice, we will reschedule your activity.
